Founded in 2024, Axiom Home Healthcare Agency is headquartered in Cincinnati, Ohio. We understand the importance of providing exceptional care to our clients and their families, and that starts with having a well-organized and efficient management system. Our aim is to simplify our agency's operations, enabling us to focus on what truly matters, delivering unparalleled care to those who need it the most in our communities. Our dedicated team of Direct Service Professionals
provide personalized support and assistance to individuals with complex conditions, empowering them to live independently in their own homes as long as possible. Axiom Home Healthcare fosters autonomy and nurtures meaningful connections, ensuring our clients receive the care they need in the comfort of their natural surroundings. Currently, Axiom Home Healthcare offers non skilled home care services in Hamilton, Butler, and Montgomery Counties. Our Direct Service Professionals are screened, bonded, insured, experienced and must pass rigorous employment qualifications. We are committed to provide quality care and constantly aim to maintain customer satisfaction.
The goal of Axiom Home Healthcare is to provide the highest quality of services to its clients while also empowering those clients to improve skill sets that will help them be able to live their lives to the fullest. Axiom Home Healthcare PHILOSOPHY:
A. SELF DETERMINATION
We believe that individuals and their families should be the primary decision makers in the management of their lives, pursue what is important to them and have a meaningful role in the community. This includes that individuals have control over the set amount of funding used to obtain the supports they need, have support to organize resourced in ways that are life enhancing, and have a circle of supports made up of family, friends, and both paid and natural supports. We also believe that individuals should be recognized for who they are and what they can contribute, having a leadership role in developing policies that affect their lives and helping others reach success. B. PERSON-CENTEREDNESS
We promote the belief that people with developmental disabilities are people first. We use a person-centered planning process to discover what is enormously important to and about a person and what talents and skills that individual possesses. This ongoing process is values-based and focus on a positive vision for the future of the person based on his/her strengths, preferences, and capacities for acquiring new skills and abilities. We also believe that service coordination is the accepted format for the provision of needed support and services. Pursuant to code: Rule 5123:2-2-01
Our specialized caregivers provide personalized care for patients with Alzheimer's and dementia, including memory care and assistance with daily living activities.
Our team provides safety assessments and fall prevention strategies to help seniors stay safe and independent in their homes.
Our caregivers can help with meal planning, grocery shopping, and meal preparation, ensuring seniors have nutritious and delicious meals.
Our caregivers can help seniors manage their medications, ensuring they take the right medications at the right times.
We provide Escort and Transportation to appointments, social events, and errands, ensuring seniors can stay active and engaged. - Proof of active driver’s license and general liability vehicle insurance is required if provider is physically driving the client.
Our caregivers assist with Activities of Daily Living (ADL) - bathing, dressing, feeding, toileting, combing hair, brushing teeth, grooming, helping seniors maintain their independence and dignity.
While a family caregiver is away, Axiom's professional caregivers with autism experience can come into the home to provide care, supervision and support for the individual with autism while the family caregiver(s) take a break. A stable and predictable environment is important for many individuals with autism and for them, in-home respite care has the advantage that they stays in familiar surroundings and can keep their normal routines through the respite period.

Our caregivers are available 24/7 to provide support and care to our clients. Our caregivers are always just a phone call away, and we are committed to providing prompt and reliable services.

Direct Support Professional (DSP) Initial Training
Completing this program satisfies the department-provided training requirements for first-time DSPs listed in OAC 5123-2-08 and 5123-3-01.
** If you are a DSP who is certified and currently providing services, do not sign up for this Program.
Certified DSPs should sign up for one of the Direct Support Professional Annual Training Programs.
See OAC 5123-2-08 and 5123-3-01 and the associated appendices for more information.
When you complete the Program, a copy of your certificate will be emailed to you and will also be available in your Dashboard under "Current Certificates."

4. Put "Completed Training Certificates" in the subject line of your email.
5. You will be responsible for completion and purchase of your BCI/FBI Background Check and First Aid/CPR certification.
